Open rhinoplasty is a surgical technique used in nose reshaping procedures. It is also known as external rhinoplasty. In open rhinoplasty, the surgeon makes a small incision on the columella, which is the strip of skin separating the nostrils. This incision allows the surgeon to lift the skin and soft tissues of the nose, providing better visibility and access to the nasal structures.

The advantage of open rhinoplasty is that it allows the surgeon to have a direct view of the nasal anatomy, making it easier to make precise modifications. It is particularly useful in complex cases or when extensive reshaping of the nose is required.

Here is a step-by-step overview of the open rhinoplasty procedure:

1. Anesthesia: The patient is typically given general anesthesia or local anesthesia with sedation to ensure comfort during the surgery.

2. Incision: The surgeon makes a small incision on the columella, the skin between the nostrils. Additionally, internal incisions may be made inside the nostrils, depending on the specific goals of the surgery.

3. Access and visualization: The skin and soft tissues of the nose are gently lifted, providing a clear view of the underlying nasal structures, including bone and cartilage.

4. Modification: The surgeon can then sculpt and reshape the nasal bones and cartilage to achieve the desired aesthetic or functional changes. This may involve removing or adding tissue, straightening a deviated septum, refining the nasal tip, or adjusting the nostril size.

5. Closure: After the necessary modifications are made, the skin and soft tissues are repositioned and sutured back together. The incisions on the columella and inside the nostrils are closed using fine sutures.

6. Recovery: The patient will be monitored in a recovery area and may be provided with nasal packing or a splint to support the nose during the initial healing phase. Swelling and bruising are common after the surgery but will gradually subside over time.

It's important to note that open rhinoplasty is just one technique used in nose reshaping procedures. The choice between open rhinoplasty and closed rhinoplasty (where incisions are made only inside the nostrils) depends on the individual's specific needs, nasal anatomy, and the surgeon's preference and expertise. Your plastic surgeon will recommend the most suitable approach based on your unique circumstances.
